• WHAT TO BRING ON YOUR FIRST VISIT

    • Drivers license or valid photo identification.
    • Insurance Card or HSA card for COVID-19 rapid test.
    • Payment for service, this includes topical numbing cream as applicable.
    • If applicable, any relevant medical records or other studies, including lab work.

    CANCELLATION POLICY
    We require 24 hours notice for cancelled appointments.

    PAYMENT POLICY
    Payment is due at the time service is rendered. Package total is due at the time of first treatment in the package.
    We accept most major credit cards. We also offer CareCredit Financing for medical spa services.

    INSURANCE CLAIM
    COVID19 EXCEPTION: We will bill your insurance or HSA for the COVID-19 rapid test.

    We DO NOT bill insurance for any medical spa services. We are more than happy to provide you with your treatment record and billing so that you can claim services to your insurance company. We do not, however, research whether services are covered or provide any assistance with the billing process.

    REFUND POLICY
    Please be advised that all payments made towards LumiereMD services and products are considered final and non-refundable. We do offer office credit for any refund requests. While the goal of all treatment is to achieve the best outcome results are individualized, and they are not guaranteed.

    OTHER OFFICE POLICIES

    Numbing Cream
    We offer a triple anesthetic cream (benzocaine/lidocaine/tetracaine) upon request for an additional charge of $12.00 per application; $15 for large areas such as legs and back. We strongly recommend it's use with IPL/photofacial treatments. You will need to apply it 30-45 minutes prior to your appointment. Please be advised that we may not be able to provide numbing cream on your first visit in order to avoid conflicts with the schedule.

    No Childcare Is Provided
    Please be advised that children should not be left alone in the waiting room during your appointment. If you have a child under the age of 8 years of age, please be sure to have an adult with them to wait while you are in your appointment. Children are not allowed in the treatment room.

    Laser Treatments on Darker Skin Types
    This notice pertains to those with a Skin Type of 4 or greater (ex. African Americans, Middle Eastern, Asian, Latino, etc.):

    Your skin color due to melanin levels is deemed to have an increased risk for first to third degree burns with laser treatments. There is an increased risk for scarring and hyperpigmentation. At any sign of pain or hyperpigmentation you must call the office immediately. We will help you with your injury. Failure to immediately report injury increases the risk of adverse side effects.

    For IPL treatments you may be required to use hydroquinone prior and during treatment to reduce your risk for hyperpigmentation. If we find that the risk outweighs the benefits you may not be able to proceed with certain laser treatments.

    IN CASE OF INJURY
    It is your responsibility to report any adverse effect (pain, tingling, redness) immediately to our office. We are here to help you. Timely reporting of potential side effects can help to reduce or eliminate injury. Failure to report increases the chances of scarring, hyper or hypopigmentation, length of time before healing, and the potential for permanent injury.

    Call 911 or go to the nearest emergency room.
